Veterans x SWCAA

SWCAA supports and salutes the many Veterans residing in southwestern Connecticut.  We are committed to supporting veterans, their families, and their caregivers by connecting them to federal, state, and community resources. Our goal is to ensure veterans receive the benefits, healthcare, and assistance they have deserve.

Our Aging Answers department has insight into VA programs such as Aide and Attendance, health care services, caregiver support and the Veterans’ Directed Care Program.  Other federal and state services can complement Veterans care and add to the quality of life for Veterans and their families.  Because social isolation and loneliness may pose risks for Veterans, SWCAA actively promotes the 988 Crisis line.  Call us at 800-994-9422 to learn more.

Veterans Crisis Line – 988 then press 1SWCAA provides comprehensive assessment, care planning, and ongoing oversight for thousands of Medicaid‑eligible adults, all with the goal of maximizing independence and enhancing quality of life. Our team of experienced social workers help individuals navigate Medicaid enrollment for the Connecticut Home Care Program, which offers community-based supports as an alternative to nursing home placement for those who meet financial and functional eligibility requirements.

Resources

The Aid & Attendance benefit is a supplemental monthly payment for eligible veterans and surviving spouses who require help with daily activities such as bathing, dressing, or eating. It can also support individuals who are blind, require nursing home care, or need assistance in an assisted living community. This benefit requires detailed documentation. Application information is available through the VA or at Veteran Aid.

The VA Caregiver Support Program provides training, resources, and guidance to caregivers supporting veterans at home. Each VA Medical Center has a Caregiver Support Coordinator who can assist with equipment needs, home modification resources, respite options, and caregiver education. The Veterans Advocacy & Assistance Program helps veterans and their dependents understand and access the benefits they are entitled to, including healthcare, financial support, housing programs, and service-connected benefits. Staff guide veterans through applications, documentation, and referrals to the appropriate VA departments.  Contact the CT Department of Veterans Affairs – Bridgeport Office (203) 576-8348.

Veterans Directed Care Program- Eligibility is determined by the West Have VA Care Coordinator.  This program may support self-directed care for Veterans with personal care needs.  Call SWCAA’s Aging Answers Department at 800-994-9422 for more information.
