Provider Enrollment

SWCAA truly values the hard work and efforts of our Providers. Provider enrollment is no longer facilitated by the Access Agency. Providers interested in enrolling or reenrolling in a Connecticut Waiver Program including the Home Care Program for Elders, the Acquired Brain Injury Waiver and the Personal Care Attendant Waiver must follow this link and select Provider Enrollment to access the enrollment instructions. SWCAA can not do business  with any Provider who fails to enroll and re-enroll as required by the State of Connecticut, Medical Assistance Program.

Once enrolled you are placed into provider rotation. It is important to note that clients are given first choice of which providers they wish to utilize. Provider rotation is utilized by staff when a client does not have a preference.  SWCAA can not guarantee a minimum number of referrals. In some cases, depending on the number of clients in a category, there could be long spans of time between referrals
