Programs for Persons with Disabilities Overview

The Southwestern CT Agency on Aging (SWCAA) administers 4 programs through the Community Options Department that are designed to help individuals who need support to live at home or return to community living. Programs are funded through the Department of Social Services (DSS) and Medicaid and allow for services to persons of all ages.  Eligibility for the programs are determined after an Initial Assessment is completed by a Specialized Care Manager. Program eligibility varies, follow the links below for program requirements and details.
