Care Manager – Social Service Professional to help older adults stay in their own home. Assess client’s needs, coordinate and authorize social services and work as part of a multi-disciplinary team to advocate and support client goals. Experience with evaluation, care plan development and person-centered goal planning required. Ability to drive throughout Fairfield County and basic computer skills required. Positions available for the Bridgeport area, Spanish speaking preferred. Required Bachelor’s degree in, Social Work, Human Services, Nursing or related field and one year of experience.
