Dietary Aide – Part Time – NDTR Required – Exciting opportunity for a community-based Registered Dietetic Technician to help educate individuals in the Home Delivered Meal Program throughout the 14 southwestern CT cities and towns. This part-time position will intake potential clients for the meals, establish eligibility and educate caregivers and clients. The Nutrition Educator will also monitor community meal sites and home delivered meal routes in the region. Flexible, 20 hours/week. Combination of office and community-based work. Transition Coordinator – Assist individuals in transitioning from nursing facility care to the community as part of Money Follows the Person, Connecticut’s Medicaid rebalancing program. Entry level position for someone with broad range of social service skills along with organizational skills to manage complex housing, health and financial documentation. Firm commitment to the policies of self determination and person-centered care. Bachelor’s Degree in Human Services or related field.
