Current Positions:


Bi-Lingual (Spanish/English) Care Manager – in the Bridgeport region, requires a Bachelor’s degree in Social Work, Human Services, Nursing or related field and 2 years of experience in Human Services. Help elderly adults stay in their own home instead of having to go to a nursing facility. Assess client’s needs, coordinate and authorize social services and work as part of a multi-disciplinary team to advocate and support client goals. Experience with evaluations, care plan development and person-centered goal planning is required. Ability to drive throughout Fairfield County and basic computer skills required.
